2025 Centre Circle Awards Nomination Form
Thank you for taking part in the Centre Circle Awards Nomination process. Ontario Soccer is continually celebrating the great work of its membership, and this Awards program is a great way to recognize the many contributions of our incredible soccer community.

Award Nominations must be submitted by Monday, March 31st, 2025. Nominations may be submitted by Clubs, Member Organizations, Ontario Soccer Standing or Operational Committees, or by an Officer or Director of Ontario Soccer.

Nominations for MilkUP Future Leaders Awards may also be submitted by a peer between the ages of 15 and 18 years old.

Ontario Soccer is committed to fostering an inclusive and accessible environment; as such, Ontario Soccer will strive to select award recipients that reflect the diversity of the soccer community.
