As per Ontario Soccer Policy 5.3.6.1, the accused, named above, would like to request a Discipline Hearing.

The accused shall submit this form to Ontario Soccer to hear the case, within 3 days of receiving the dismissal. This request must arrive before the date of the next scheduled Discipline Review meeting, as published.

The Request for a Hearing Fee is $250.00 and must be paid when this form is submitted. Please note that a hearing will not be scheduled until payment is received. This fee is non-refundable.
