You are a Match Official for Ontario Soccer.  By submitting this form you agree to the following:

• You will invoice Ontario Soccer for your services
• You will NOT receive a T4A for tax purposes in February, even if their total payments have exceeded $500

The Match Official/Service Provider is responsible for reporting their own income to the Canada Revenue Agency.

This form will be submitted in January of the current calendar year.  It must be submitted annually.  You may not change back until the start of the next calendar year.

As a match official who appears on the list of registered match officials for Ontario Soccer assigned program events, I agree to provide match officiating services for Ontario Soccer under the following terms and conditions:

1. Ontario Soccer is recognized as the Provincial Governing Body for the Registration, Accreditation, Development and Management of Match Officials. I acknowledge that all assignments made by Ontario Soccer to competitions under its jurisdiction or on behalf of any league, promoter or other recognized sports body are subject to the provisions outlined in their Regulations or Operational Procedures.
2. I shall not officiate in any Competition or League, which is not Ontario Soccer sanctioned, or a match in which unaffiliated clubs/teams are participating.
3. I reserve the right to accept or decline assignments at my sole discretion.
4. It is acknowledged that Ontario Soccer makes assignments at its sole discretion and that this agreement does not guarantee any minimum number of appointments.
5. I will inform Ontario Soccer of dates for which I am unavailable - via its preferred method, and reserve the right to decline an assignment on a date for which I had otherwise stated I was available on.
6. I will invoice Ontario Soccer for services provided on a monthly basis. My invoices will include:
   
   1. My full legal name or business name
   2. League matches officiated (include league name, game number, date, location and teams)
   3. Game fee amount (as per approved Ontario Soccer rates)
   4. Additional expenses if applicable (i.e., travel as per Ontario Soccer approved rates, accommodations with original receipt attached)
7. Invoices are subject to payment within 30 days of the invoice receipt date and I will enroll in direct deposit by electronic funds transfer to receive payment for invoice.
8. I understand that it is my responsibility as an independent contractor to declare my income derived from match officiating services to the Canadian Revenue Agency (CRA) and I will hold Ontario Soccer harmless from prosecution or penalty resulting from assessments on my account by the CRA.
9. I am responsible to maintain my own equipment and uniforms for any assignment, as required by the CSA or Ontario Soccer for that specific assignment or competition.
10. I am responsible for maintaining my own physical fitness, and acknowledge that I am required to attend and successfully achieve any fitness test standard as outlined by Ontario Soccer, for the purpose of promotion, recertification, or reinstatement.
11. It is understood that a pyramid of assigning for officiating exists, and that certain matches take precedence over others. Should I decide to accept an appointment on any given date, I agree to accept the appointment higher in the pyramid of assigning as outlined in the CSA Regulations for the Registration and Control of Match Officials.
12. I agree to uphold the standards, values and reputation of Ontario Soccer, as stated in their Code of Ethics or provided to me from time to time in respect to specific competitions.
13. I will complete and file match sheets and/or incident reports within 72 hours (3 days) of the completion of each game.

This agreement will remain in effect until such time that I am provided with new term and conditions.
