Using this form will automatically generate an email to you, your current club, and your District Association. You will first receive the selected form to complete. Once submitted, it will be forwarded to your club and then to the District Association for the required approvals. After the process is completed, you will receive a copy for your records.
If you select the Transfer Form, you may provide the completed form to your new club. If you select the De‑Registration Form, please retain it for your personal records.
