Description: This form must be completed and approved when a player is looking to transfer from one Club to another Club. Players must be in good standing with the Club they are transferring from.
Applicable Operational Procedures: Section 2 Registration; Procedure 13.
Before using this form you will need to know the Club Contact Name and email address at the releasing Club and the new Club. The contacts need to be a person in authority that can approve a transfer, typically the Club Administrator and not the teams coach.
Once the transfer is approved by the releasing Club and District, the new Club and District will be notified by email and sent a copy of the approved transfer form so that they may register you.
