Ontario Soccer De-Registration Form
Description: This form must be completed by the Player/Parent/Guardian and approved by the Club and District when a player wants to De-Register from their current Club. Players MUST be in good standing with the current Club if they want to register again at a later date within the same season.

Applicable Operational Procedures: Section 2 Registration; Procedure 10, Procedure 13.2 d)

Before using this form you will need to know the contact name and email address of the Club Administrator at the releasing Club. The contact needs to be a person in authority that can approve a de-registration, typically the Club Administrator and not the team's coach.

Players wanting to move from their current Club to a new Club must use the Player Transfer Form (go back to the Forms webpage and choose the Player Transfer Form instead).

If you simply want to end your registration with Ontario Soccer (more specifically, with this Team and Club) this is the correct form.

You are not De-Registered until both the Club and the District Association approve this form. You will receive a copy by email once it is complete.
