OPDL Event Permission Request Form
This form is for OPDL Licence Holders to request approval for their teams to attend any event not included in the list of pre-approved events. This form must be completed prior to applying to enter the event to avoid unnecessary logisitc challenges if denied.

Please note: OPDL teams are not permitted to attend events in-season and players selected to participate in the Provincial Xcel Program are not permitted to attend conflicting events as per OPDL Operational Rules.
